Home evaluations of patients with a physician's order for home speech therapy services will include, but not limited to, the following:

Our speech therapists can:

• Complete the physician's Plan of Treatment for speech therapy, including the amount, frequency and duration of specific procedures and modalities.
• Set realistic measurable short and long-term goals with the patient and family.
• Assess the patient's progress and ability to tolerate the prescribed treatment program.
• Re-evaluate the patient's speech therapy needs and implement physician ordered changes.
• Check home environment, adaptive language equipment and devices or oral prostheses for safety and proper working order.
• Compile documentation of visits reflecting skilled care as defined in plan of treatment.
