Nursing Homes – Our Smooth Transitions Program will take the burden off your staff in transitioning the patient safely home. 
 
Black Stone’s Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Therapists (PTs, OTs, speech), and Social Workers (SWs) provide services in the home that cannot be performed safely or effectively by personnel lacking the education and experience of these professionals. Referred to as “skilled services,” these services differ from the non-skilled tasks that may be provided by a nurse’s aide or family caregiver, such as assistance with bathing, feeding, toileting, etc. If you or a patient is in need of non-skilled services, MJ Nursing by Black Stone can arrange the care you require.
 
Because of the medical necessity of these services, Medicare and many other insurances will pay for skilled care in the home as long as specific criteria is met. Skilled services fall into broad categories such as assessment of patient, teaching of self-care to patient or primary caregiver, specific interventions such as wound care or physical therapy, injections and intravenous therapy, and coordination of care with other health care providers including physicians.
