Black Stone works with your patients to help them manage COPD and/or CHF

 

33.5% of Medicare home care patients have conditions related to diseases of the circulatory system as their principal diagnosis. Because of the large population with COPD and CHH, Black Stone Home Healthcare specializes in disease management and education for those common health issues. 
 
We follow your treatment plans to help relieve symptoms and assist your patients with medications to lessen the exacerbations in COPD sufferers. Our nurses are, of course, familiar in the use of bronchodilators and inhalers. We can arrange oxygen treatments and nebulizers for patients and teach them how to properly use the equipment. In fact, our patient education booklet, “Managing COPD,” outlines instructions for using and cleaning the nebulizer, along with instructions on how to clear lungs, proper diet, and other helpful information for COPD patients. Ask your Black Stone liaison for a copy.
 
Our “Managing CHF” patient education booklet explains ACE inhibitors, diuretics, and Digoxin, reinforces a heart-healthy diet, and provides daily weight charts. During visits, our nurses monitor medications and blood pressure while reinforcing the benefits of lifestyle changes, such as quit smoking, reducing salt, losing weight, and exercising. Your Black Stone liaison will be happy to provide you with copies of the booklet used to educate your patients.
