Geriatric Care Management Enable Independence and Improves Quality of
Life Among Aging Adults
Cincinnati, OH – August 15, 2011 – Home Care by Black Stone has been providing professional care to elderly and disabled individuals living in Southwest Ohio since 1996. Black Stone’s services allow elderly individuals to remain in the comfort of their home while receiving periodic or ongoing care. Care Advisors by Black Stone, established in early 2010, provides a professional advisory resource for seniors and their loved ones as they navigate the complex and often confusing aging process.
David Tramontana, CEO of Home Care by Black Stone stresses the importance of being proactive as seniors age. "As the baby-boomers reach retirement age, the population of individuals aged 65 and above continues to grow, having already swelled by 5.3 million since the 2000 U.S. Census Report. The AARP is estimating 60 million households will be providing care for an adult over the age of 50 within 25 years. Care Advisors by Black Stone aims to assist seniors and their family members in developing a care plan, which will ease the transition as they approach older adulthood. We provide the guidance and knowledge necessary for families to take control of the aging process."
Care Advisors by Black Stone enhances the lives of aging individuals as well as their family members. The program, which is staffed by registered nurses, encourages seniors and loved ones to participate in the care planning process. Geriatric care management services begin with an overall assessment of the aging adult’s medical, financial, social, psychological, and environmental needs. The resulting, customized care plan can help the seniors and their families understand their options and make informed decisions about the future. By encouraging seniors to take part in this process, they retain a sense of their independence because they are still actively participating in the planning.
“Our registered nurses aim to connect with the seniors and be their health advocate,” said Micki Fehring, RN and geriatric care manager for Care Advisors by Black Stone. “It is not uncommon for older adults to be embarrassed to tell their family or doctors that they can no longer take care of themselves or their spouse. In addition, it can be challenging to navigate the complex treatment plans and coordinate the care of multiple health providers. Through Care Advisors, Black Stone strives to make sure seniors, their loved ones, and physicians are on the same page and prepared for the future.”
Care Advisors by Black Stone equips families with the tools necessary to face challenges that arise as loved ones age. Research by Cornell University’s Project Home shows that a patient-centered plan can enable seniors to remain at home when there is focus on the their individual needs. Services from Home Care by Black Stone ensure individual attention and a professional assessment, which will result in a successful eldercare plan, allowing seniors to remain at home and retain their independence and happiness.
